The Cambridgeshire and Peterborough Child Death Overview Panel (CDOP) was established on 1st April 2008 in accordance with the Children Act 2004 and statutory guidance Working Together to Safeguard Children.
The Child Death Overview Panel is responsible for collecting and analysing information about the death of every child under 18 years old in Cambridgeshire and Peterborough, with a view to:
- reduce the numbers of child deaths
- prevent accidents to children
- identify of and understand the patterns of childhood death
- improve the interagency practice in this very sensitive area
- educate the public and professionals working with children
- highlight any matters of concern
- identify the need for a Serious Case Review
It also has the job of overseeing the process of conducting a rapid response by a group of key professionals to enquire into and evaluate each unexpected death of a child. This applies to all babies/children resident in Cambridgeshire and Peterborough, even if they are out of area at the time of death. Deaths of children who are normally resident in another area should also be reported and the information will be passed onto the relevant co-ordinator once a decision has been made as to who will co-ordinate the information gathering.

Any agency becoming aware of a child death occurring in Cambridgeshire or Peterborough, or a death of a normally resident Cambridgeshire or Peterborough child occurring elsewhere should notify the Cambridgeshire and Peterborough Child Death Review Manager Natalie Jones via email CAPCCG.email@example.com.
Notifications should be made on Form A – Notification of Child Death.
Following notification of the death of a child, the Child Death Review Manager will establish which agencies and professionals have been involved with the child or family either prior to or at the time of death by contacting the lead professionals in each agency.
The Agency Report Form B will then be sent by the designated person to the lead professional in each agency and to any professionals known to have been involved for completion.
